Sensitive index of renal function
Abstract. A sensitive single measure of diminishing renal function is of importance in attempts to modify the progression of diabetic nephropathy. In 12 insulin-dependent diabetics with proteinuria plasma concentrations of beta 2-microglobulin were found to correlate more closely than plasma creatinine concentrations or creatinine clearance Renal function must be adequate to avoid complications such as pulmonary edema, and the patient must be monitored for derangements in systemic acid–base balance or electrolyte balances. The renal arterial resistive index (RI) is a sonographic index to assess for renal arterial disease. It is measured as. RI = (peak systolic velocity - end diastolic velocity ) / peak systolic velocity. the normal value is ≈ 0.60. with 0.70 being around the upper limits of normal. Moderate decrease in renal function, with or without other evidence of kidney damage. Observation, control of blood pressure and cardiovascular risk factors. IV: 15-29: Severely reduced renal function. Planning for end-stage kidney disease. V <15: Very severe (end-stage) kidney disease. Transplant or dialysis.

Urinary concentrations of N-acetyl-glucosaminidase (NAG) and β 2 -microglobulin (β 2M) are sensitive indices of renal tubular function. NAG is a lysosomal enzyme present in proximal tubular cells.4 Its molecular weight precludes filtration at the glomerulus but it is rapidly cleared from the circulation by the liver. An assessment of renal function may be required for several reasons: To identify renal impairment. To monitor disease progress. To assess baseline measurements prior to starting treatment with certain drugs. To monitor disease progress. The type of measurement of kidney function performed will be determined by the reason for assessing renal activity. A sensitive single measure of diminishing renal function is of importance in attempts to modify the progression of diabetic nephropathy. serum creatnine level in renal dis.generally do not increase until renal function is substantially impaired. determination of creatnine clearance is a highly sensitive test for measuring the GFR is reabsorbed in the renal tubules. –Urinary BMG levels are a sensitive measure of renal tubular function uIncreased in renal failure Tests of Tubular Function performed less frequently uProximal Tubular Function –Aminoaciduria –Glycosuria with normal blood glucose uDistal Tubular Function Dr. Khalid Al-Ani Our Renal Function Test Panel includes ten measurements and two calculations that provide valuable information necessary to determine your kidney health. Your kidneys filter over 180 liters of blood every day, removing waste and excess water. Without your kidneys filtering ability, the unwanted waste and water would accumulate in your blood and cause damage. In the early stages of chronic kidney disease, you may have few signs or symptoms. Chronic kidney disease may not become apparent until your kidney function is significantly impaired. Treatment for chronic kidney disease focuses on slowing the progression of the kidney damage, usually by controlling the underlying cause.
the other is to detect the presence of a progressive loss of renal function. Diabetic Serum creatinine is a more sensitive index of kidney function compared to.

Conflicting data have been reported concerning the use of renal arterial resistive index to predict future events, i.e. loss of renal allografts and deaths. The Doppler-derived renal resistive index has been used for years in a variety of clinical settings such as the assessment of chronic renal allograft rejection, detection and management of renal artery stenosis, evaluation of progression risk in chronic kidney disease, differential diagnosis in acute and chronic obstructive renal disease, and more recently as a predictor of renal and global outcome in the critically ill patient. Renal function is an indication of the kidney's condition and its role in renal physiology. Glomerular filtration rate ( GFR ) describes the flow rate of filtered fluid through the kidney. Creatinine clearance rate ( C Cr or CrCl ) is the volume of blood plasma that is cleared of creatinine per unit time and is a useful measure for is reabsorbed in the renal tubules. –Urinary BMG levels are a sensitive measure of renal tubular function uIncreased in renal failure Tests of Tubular Function performed less frequently uProximal Tubular Function –Aminoaciduria –Glycosuria with normal blood glucose uDistal Tubular Function Dr. Khalid Al-Ani Serum creatinine levels and body weight were measured just before the start of treatment and at the end of treatment or the current level if still on treatment. Factors known to effect renal function, specifically diseases such as hypertension, chronic renal impairment, heart failure and diabetes were documented.
